DFW Urgent Care offers discounted rates to all private pay patients. We also have a discount membership plan
available for patients and their families without medical insurance as well as those insured patients with high
deductibles or lapse in coverage. With proof of membership, a patient can be seen at any DFW Urgent Care facility
at a reduced cost.
The Discount Membership Plan is valid for all provider services provided by DFW Urgent Care. The Discount
Membership Plan includes spouses and all legal dependents identified in the Member Application. It is NOT
insurance; it is a discount card that can be used for DFW Urgent Care medical services.
PROOF OF MEMBERSHIP – In order to use your Discount Membership, you must present your
Discount Membership Card, along with valid photo identification at the time medical services are rendered.
CANCELLATION POLICY – The Discount Membership Plan may be cancelled with a full refund of
membership fees (excluding the application fee) within the first 30 days of purchase. However, if you have purchased
your membership during a DFW Urgent Care visit and you give written notice of cancellation within 30 days, your refund
will be less the discount given at the time of service. This situation may require you to pay a portion of the discount
you received at time of service.
SIGN UP – To become a member of our Discount Plan you can stop by any DFW Urgent Care location to pick
up an application, you can print out an online application and fill it out before your next visit, or you can sign up online.
